Private Medicals & Reports 
If you require a full medical examination, medical report or letters for driving, housing, insurance or employment you will be charged at BMA recommended rates. These include insurance claims, applying for mortgages, fitness to work and many more. The fees we charge are in line with the guidelines set by the BMA and for a full list of services please ask at reception.


Your Rights 
You have the right to information about your own health including; 
  • Prevailing illnesses and treatments 
  • Prevention of ill health 
  • Proposed investigations 
  • Right to Confidentiality 
  • To see your medical records subject to (Data Protection act 1990) 
If you wish to view your notes please make a formal request to the Practice Manager. A fee is normally charged for this service.


Confidentiality  
We Respect your right to privacy and keep all your health information confidential and secure. It is important that we keep accurate and up-to-date records so that all those involved in your treatment can give you the best advice and care.


Choose & book referrals
If the GP decides that you require to be referred to hospital you will be offered a number of hospitals you may attend at. If you have a particular preference of hospital please inform the GP. We will endeavour to give you a date and time for the appointment before you leave the surgery. 


What We Ask of You 
We will endeavour to provide the best service we can for all our patients. However, we request you help in order to achieve this. We urge you to :
  • Treat the health care team with courtesy and respect 
  • Be patient and understandable when the staff are busy 
  • Cancel unneeded home visits as soon as possible 
  • Not to call the doctor unless essential 
  • Plan ahead with repeat prescriptions and vaccinations 
  • Keep us up-to-date with changes of address or telephone numbers 
  • Keep children under control 
  • Please keep the surgery clean and tidy 
  • Keep mobile phones switched off at all times 
  • Please do no eat, drink or smoke on the surgery premises

Suggestions, Comments & Complaints 
If you have any suggestions or comments that can help us develop and continually improve our service please feel free to write your thoughts down and give it to the Practice Manager or leave it in the suggestions box at reception. If you have any concerns about any aspect of our practice please let us know. Speak to whomever you feel comfortable with – your GP, our Practice Manager or our reception staff will be happy to help. If you felt that your complaint needs to be heard officially, please write it down and address it to the Practice Manager. Your complaints will be dealt with according to our formal complaints procedure.
